Gym Policies + Waiver Forms

Every season (July 1 – June 30), we require our members to complete a new set of waiver forms. For regular registered programs, waiver forms are accepted through the student policies in iClassPro. Waiver forms must be accepted in your online account before the first day of gymnastics.
 
Any adults, birthday parties, user groups, gym rentals, etc., participating in gymnastics will need to complete and submit the paper Release of Liability and PIPA forms before entering the gym.

Child Waiver

Minor (Under 18) Release of Liability for Participation in Gymnastics. Form must be initialled and signed by the guardian of the participant.

Personal Information Protection Act (PIPA) Form. To be filled out for all participants.
